Chad Wesley Campbell

Passed away in Children's Hospital in Birmingham. Visitation will be held at Nichols Funeral Home in Addison with extended hours of 3:00 to 9:00PM on Monday. Services will be from the funeral home chapel Tuesday, 2:00PM with burial to follow in Chance's Crossroads Church of Christ Cemetery. Chad is survived by his parents Randall and Rhonda, brothers Chase, Myles and Tyler Campbell. Also survived by Grandparents Charles & Elouise Campbell and Coleman & Jackie Freeman and a Great Grandmother Rosabelle Campbell Howlett
